Cal State Dominguez Hills Athletics

The following facilities are available for rent at Cal State Dominguez Hills:
- Torodome (Gymnasium)
- Toro Field (Baseball)
- Toro Diamond (Softball)
- Activities Field (Multi-purpose)

We need terms 1, 2 & 3 returned quickly.  Upon receipt of all three items, a contract will be formulated and at that time you can sign and return with a check for the full amount no later than 5 pm one week before the date of the event.

1.  Facilities Request Form Filled out, signed and returned to me. Request form here.
 
2. Certificate of Insurance: An example of  copy of an acceptable certificate and endorsement page can be downloaded here.  Under "Insured" is your name and address, and under "Certificate holder" is California State University Dominguez Hills, 1000 East Victoria Street, Carson, CA 90746.  The policies should match on each page.
 
3. Endorsement Page with the CSUDH wording on it. Please copy and paste the ENTIRE box or use the form here.

4. Upon reception of the request form, certificate and endorsement page, the paperwork will be processed and a contract formulated which will need your signature and payment.
 
*Parking cost $8 per day.

For outside vendors we need a Public Health Operating Permit, Sellers Permit, Tax Registration, and the approval from off-campus catering, with the form available here that needs to be filled out completely and returned to me at the address below.
